Data Subject Application Form

1. General Information on Your Right to Apply

• Under Article 11 of the Personal Data Protection Law No. 6698 (“KVKK”), a natural person whose personal data is processed (the “data subject”) has the right, by applying to the data controller, to:
  1. learn whether their personal data is processed,
  2. request information if their personal data has been processed,
  3. learn the purpose of the processing of their personal data and whether it is used in accordance with that purpose,
  4. know the third parties to whom their personal data is transferred, in Türkiye or abroad,
  5. request the rectification of personal data that has been processed incompletely or inaccurately, and request that the operations carried out in this scope be notified to the third parties to whom the personal data has been transferred,
  6. request the erasure, destruction or anonymisation of personal data where the reasons requiring its processing no longer exist, even though it was processed in accordance with the KVKK and other applicable laws, and request that the operations carried out in this scope be notified to the third parties to whom the personal data has been transferred,
  7. object to a result arising to the person’s detriment through the analysis of the processed data exclusively by automated systems,
  8. request compensation for damage suffered due to the unlawful processing of personal data.
• You may also submit your requests under Article 11 of the KVKK to us by using this Application Form, which we have prepared within the framework of Article 13 of the KVKK and the Communiqué on the Procedures and Principles for Applications to the Data Controller.
• This Application Form has been prepared in order to identify your relationship with us, to determine completely the personal data of yours that we process, if any, and to respond to your application accurately and within the statutory period.
• Applications made under Article 13 of the KVKK may be subject to a fee in accordance with Article 13/2 of the KVKK and Articles 6/5 and 7 of the Communiqué on the Procedures and Principles for Applications to the Data Controller.
• Applications submitted to us will be answered as soon as possible and at the latest within 30 (thirty) days, depending on the nature of your request, in accordance with Article 13/2 of the KVKK. Our responses will be delivered to you in writing or electronically, in accordance with Article 13/3 of the KVKK.
• In order to eliminate the legal risks that may arise from unlawful and unjust data sharing, and in particular to ensure the security of your personal data, we reserve the right to request additional documents and information (such as a copy of an identity card or driving licence) for the verification of identity and authority.
• We accept no liability for requests arising from incorrect information or unauthorised applications where the information you provide regarding your requests under this form is not accurate and up to date, or where you make an unauthorised application.

2. Application Method

You may submit your application containing your requests under the rights listed in Article 11 of the KVKK to us by using this Application Form through one of the methods described below, in accordance with Article 13 of the KVKK and Article 5 of the Communiqué on the Procedures and Principles for Applications to the Data Controller.
